What it is

Finasteride is the most proven oral medication for male pattern hair loss. It was first approved at a higher dose for prostate enlargement; researchers then found that a 1 mg daily tablet reduced the DHT that shrinks hair follicles, and in 1997 the FDA approved it for androgenetic alopecia under the brand name Propecia®. It does not stimulate growth the way minoxidil does. It removes the hormonal signal that is shrinking the follicle, which is why physicians treat it as the foundation of a hair-loss protocol and add a growth agent on top when needed. How it works

In men genetically prone to pattern hair loss, an enzyme called Type II 5-alpha reductase converts testosterone into dihydrotestosterone (DHT) in the scalp. DHT binds to susceptible follicles and gradually miniaturizes them. Finasteride inhibits that enzyme, lowering scalp and serum DHT by about 60 to 70% at the 1 mg dose used for hair loss. Testosterone itself is not removed; the conversion step is. Taken as a daily tablet, it is a prevention-first treatment: the earlier you start after thinning begins, the more hair you still have to keep.

What the research shows

Finasteride's hair-loss evidence is unusually clear. In the pivotal two-year trials of 1 mg daily in men with vertex balding, 83% of men on finasteride had no further visible hair loss versus 28% on placebo, and a substantial share gained hair count. A five-year extension showed the difference widening over time: untreated men continued to thin, treated men largely held or improved. Vertex (crown) thinning responds more reliably than a deeply receded hairline, which is worth knowing before you start. These trials used the FDA-approved oral product. Sexual side effects were reported by about 2% of men in those trials, slightly above placebo, and are discussed below rather than buried.

What to expect

Finasteride is slow on purpose. Most men judge it at six to twelve months, not six weeks. The first win is usually that the thinning stops getting worse; thickening, if it comes, follows. A brief shed in the first months can occur as follicles reset. Take one tablet at the same time each day; missing doses lets DHT rebound. Side effects and who it's not for

The side effect that deserves a straight conversation is sexual: decreased libido, erectile changes, or reduced ejaculate volume, reported in roughly 1 to 2% of men in the hair-loss trials. For most men who report them, they resolve after stopping; a small number of men have described symptoms that persist, which is uncommon and still studied. Other possible effects include breast tenderness and, rarely, mood changes. Finasteride is not for women who are pregnant or may become pregnant: it can cause birth defects in a male fetus, and crushed tablets should not be handled by anyone who may be pregnant. FAQ

Is this the same as Propecia®?

Same active ingredient, finasteride, at the 1 mg hair-loss dose, as a daily tablet. Propecia® is the brand; finasteride is the generic. The difference is price, not the molecule.

Will it affect my testosterone or fertility?

Finasteride does not lower testosterone; it reduces the conversion of testosterone into DHT. Most men see a small rise in measured testosterone. Effects on fertility are usually modest and reversible after stopping, but if you are actively trying to conceive, tell your physician so the decision is made with that in view.

Finasteride or minoxidil, or both?

Finasteride addresses the hormonal cause. Minoxidil helps follicles grow. Together they are the most complete medical protocol we offer, which is why the 2-in-1 tablet exists. Either can be used alone if that is the better fit for you.
