Tadalafil is the active ingredient in Cialis®, approved by the FDA in 2003 as the third entrant in the ED pill class and the one that changed how these medications fit into real life. Where sildenafil gives you an evening, tadalafil gives you a day and a half, and its low-dose daily version removes scheduling entirely. It has also been FDA approved since 2011 for urinary symptoms from an enlarged prostate, which it often improves as a side benefit. Like sildenafil, tadalafil blocks the enzyme PDE5 so that blood vessels in the penis relax and fill more easily when you are aroused. The chemistry differs in one important way: tadalafil stays active in your body far longer. A single dose covers up to 36 hours, and the daily microdose keeps a steady low level at all times, making spontaneity the default rather than the exception. As with all medications in this class, arousal is still required; it supports the response, it does not manufacture it.
For on-demand use, take it at least 30 minutes before sex; meals do not meaningfully interfere, so no dinner logistics. For daily use, one small tablet at the same time each day, and after about a week your baseline readiness is simply there. Most men respond well, and your physician can adjust dose or switch you between the two approaches as your life demands.
Common side effects are headache, flushing, indigestion, nasal congestion, and, more characteristic of tadalafil, occasional back pain or muscle aches, usually mild and passing within a day or two. The same absolute rule applies as with all PDE5 medications: never combine with nitrates, because the interaction can drop blood pressure dangerously. Mention any heart condition, low blood pressure, or alpha-blocker prescription during your physician review.
